Requirements for Admission

1. Birth certificate (original) 
2. Photo ID of parent/guardian 
3. Immunization records. Click for the list of required immunizations. 
4. Physical exam form
5. Dental form
6. Proof of residency
7. Baptismal record for students who may identify as Catholic. The record is not required for admission.
8. Report cards for grades 1-8
9. A $150 registration fee per student will be due if accepted.

If you would like assistance submitting these documents, you can bring them to Gesu’s main office. Our staff will make copies and submit the documents. Or, you can upload the documents and submit them electronically.
